Personalised Care Ambassador

Do you want to be part of an exciting team who supports the education and training for Primary Care Personalised Care teams with PCNs and General Practice across Derbyshire?

We are looking for someone well established in their role within personalised care (social prescriber, health and wellbeing coach or care co-ordinator) to join our team whose skills and experience will benefit and support all who work in these roles in general practice from new to general practice peers and beyond.

The Role

  • To be a role model for all personalised care staff to encourage integration, cross sector working and development which will aid retention of existing staff.
  • Attend wider system meetings and represent Derbyshire.
  • Share experiences, innovations and good practice with key stakeholders locally and nationally.
  • Support the development of business case/s for additional funding and support general practice workforce with collaborative working with key stakeholders.
  • Review and develop workforce skills mix within PCNs, identify the training needs of personalised care staff to support education and career pathways.
  • Enhancing the recruitment of new staff by working with the PCNs to adopt a standard approach across Derbyshire.
  • Support the transition, induction and embedding for new to general practice colleagues.
  • To help facilitate workshops and/or learning sessions for those in personalised care roles.
  • Assist in developing a workforce strategy to further embed general practice personalised care professions.

The successful candidate will possess;

  • Excellent communication skills
  • Competent IT skills
  • Knowledge of personalised care roles in general practice across Derbyshire
  • Ability to build good working relationships with stakeholders
  • Be able to engage with and provide leadership to a range of colleagues

Working pattern:

  • 4 hours a week, flexible working pattern, ideally must be available to attend key system meetings which are on Wednesdays
  • 6 months fixed term contract with protentional to extend subject to funding
  • Third party contract at £17.30 per hour

  • The closing date for this role is 3rd May 2024.
  • Face to face interviews are planned for mid-end May 2024.
